Your Privacy and Rights
Privacy
Under federal law, your patient health information is protected and confidential. Patient health information includes information about your symptoms, test results, diagnosis, treatment, and related medical information. Your health information also includes payment, billing, and insurance information.
We will need to use your information to contact your with appointment reminders and for preadmission processes. Please notify your physician and TVEC if you need special arrangements made to protect your privacy (for example, if you prefer messages not be left on answering machines).
Advanced Directives
Advanced directives and living wills are important documents. In the event of an emergency, it is our policy to resuscitate you and transfer you to an area hospital for further care. If you need to make special arrangements regarding your advanced directives, please notify us in advance.
Complaints
If you have a complaint, ask to see the administrator. This will not compromise your access to or quality of care.

Teaching and Research
You have the right to refuse student observers and to decline to participate in clinical trials.
